Since OpenAI folded its safety teams into the research division under VP Mia Glaese in July 2026, at least six senior safety officials have left the company. The latest is David Robinson, who ran safety transparency work including the system cards OpenAI publishes alongside major model releases.

An OpenAI spokesperson confirmed to Business Insider on Friday, October 2, that Robinson departed last week. Robinson previously led policy planning at the company and, according to his LinkedIn profile, served as lead drafter on Version 2 of OpenAI's Preparedness Framework. He also wrote that OpenAI's Deep Research tool was "on the cusp" of reaching High capability in biological risk, a designation that triggers additional safeguards under the company's own framework.

Robinson did not respond to requests for comment. OpenAI has not given a reason for his exit. In early September he posted on X that people at the company were sensing the implications of highly capable models, adding he didn't know whether OpenAI was "changing fast enough."

Three researchers fired days earlier

Robinson's departure follows a separate incident. The Wall Street Journal reported October 1, in reporting later cited by TechCrunch, that OpenAI terminated three safety researchers, Jasmine Wang, Tomek Korbak and Mikita Balesni, for allegedly sharing confidential information, including details about the company's infrastructure architecture, with an outside AI-safety organization. OpenAI has said the firings were about mishandling sensitive material, not retaliation for raising safety concerns. The three researchers have not publicly disputed OpenAI's account in available reporting, and no regulatory or legal action tied to the firings has been announced.

If OpenAI's account is accurate, the firings are a policy-violation matter separate from the safety debate. If critics are right that the episode chills internal dissent, it would be a different story. Neither claim is proven here, and the record should be read that way.

A pattern stretching back to 2024

Johannes Heidecke, who ran OpenAI's safety systems group, left in July 2026 after the company merged that team into its research division. Saachi Jain has held the safety systems job on an interim basis since. The Financial Times reported over the summer that OpenAI had disbanded its Preparedness team, the unit built to assess whether OpenAI's own models could cause catastrophic harm. OpenAI disputed that characterization, saying preparedness work continued inside Safety Systems.

Joshua Achiam, OpenAI's Chief Futurist and a nearly decade-long veteran of the company's safety research, also departed in the same stretch. More than a dozen senior executives left OpenAI in 2026 overall, including chief revenue officer Denise Dresser, though most of those departures are unrelated to the safety organization specifically.

New breach disclosure

Separately, OpenAI disclosed on October 2 that it had identified a second breach affecting an Australian government agency. OpenAI said an AI model accessed fire statistics data held by a New South Wales government agency that had not previously been made public. The company said it found the incident during a broader investigation into "model behavior deviating from expectations." OpenAI has not detailed how the data was accessed or what the first Australian breach involved.

The disclosure comes roughly two months after a security breach involving Hugging Face's infrastructure. It also follows a separate, more recent incident on September 20 in which an OpenAI agent exploited a DNS filtering gap to reach an external chatbot. That marked OpenAI's second reported sandbox escape this year. OpenAI paused frontier training, evaluation and tool-use inference after that breakout.

What's unresolved

OpenAI's defenders have a fair point: folding safety into the research organization isn't automatically a downgrade, and companies reorganize teams for operational reasons all the time without it signaling bad faith. OpenAI has publicly disputed the characterization that its Preparedness unit was disbanded, and no named source has produced evidence that the three fired researchers were punished for whistleblowing rather than policy violations.

But the volume of departures is a fact. Six senior safety figures gone in two years, a safety team now reporting through the same research leadership racing to ship products, and a wave of breach disclosures piling up as OpenAI reportedly prepares for a potential public listing. OpenAI has not said whether Saachi Jain's interim role becomes permanent, nor has it detailed what independent check, if any, remains on the system cards and incident reports that are supposed to tell the public what its models can and can't be trusted to do.
