Researchers led by Carmit Libruder, an epidemiologist with the University of Haifa and the Israel Ministry of Health, linked two national databases, the Israel National Cancer Registry and the Israel National Stroke Registry, to track what happened to 182,221 adults aged 40 and older after their first invasive cancer diagnosis between 2014 and 2021.

The numbers are stark. Cancer patients had an 80% higher rate of ischemic stroke than expected based on general population rates, a standardized incidence ratio of 1.8, according to the study as reported by Drugs.com and Healio. That's 62 strokes per 10,000 patients over one year.

Younger patients, ages 40 to 54, had a standardized incidence ratio of 3.6, dropping to 1.4 for patients 75 and older, according to Drugs.com. Meanwhile, risk climbed with disease severity, from 1.2 in localized cancer to 2.8 in metastatic cancer.

Why It Might Be Happening

The study was observational, meaning it can show an association but can't prove what's causing it. Libruder told Everyday Health she suspects cancer's effects on blood clotting and inflammation play an outsized role in that early post-diagnosis window.

Maarten Lansberg, a professor of neurology and co-director of Stanford's Stroke Recovery Program, who wasn't involved in the study, told Everyday Health that pancreatic and lung cancers are more likely to trigger hypercoagulability, a condition where blood gets "stickier" and more prone to clotting, which can trigger stroke.

Libruder called the early months after diagnosis "a critical window" for stroke risk in comments to Healio, and said the findings argue for a multidisciplinary approach to cancer care that doesn't lose sight of vascular health while focused on tumor treatment.

What the Data Can't Tell You

The study draws entirely on Israeli national registry data, not U.S. patients.
