The claim comes from a podcast conversation between Musk and Sacks, not from an OpenAI security disclosure or an independent audit. OpenAI has not confirmed this specific incident in any source reviewed here. The incident reportedly occurred during internal pre-release testing, according to 24/7 Wall St., which if true would mean it happened before any public release.
